Collaborative Care Program
Collaborative Care Program
The Collaborative Care Program is a virtual practice overlay, comprised of care managers and
consulting Psychiatrists, to help primary care practices integrate behavioral healthcare into the
primary care setting.
The Collaborative Care Program brings mental health and primary care together to ensure you
receive comprehensive and personalized support for your well-being. This innovative model is
designed to make mental health care accessible, effective, and integrated into your overall
